Difference Between EKG and ECG: Are They the Same Test?

EKG and ECG are the same test. Both stand for an electrocardiogram, the recording of the heart's electrical activity that a machine builds from electrodes on the chest and limbs. There is no clinical difference between the two: same procedure, same equipment, same tracing, same reading. The only reason two abbreviations exist is language. So are EKG and ECG actually the same thing?

Whether someone writes it "EKG" or "ECG," they mean that same recording, produced the same way. A patient told they need an EKG and a patient told they need an ECG are getting the identical test.

Why are there two abbreviations at all?

EKG comes from the German Elektrokardiogramm, where the "cardio" sound is spelled with a K. The German-language origins of a lot of early cardiac work carried that spelling into everyday clinical use, and it never left.
In the United States you'll hear "EKG" spoken constantly, especially at the bedside. One commonly cited reason clinicians favor the K sound out loud is that "ECG" can be misheard as "EEG," the electroencephalogram, a completely different test that records brain activity rather than heart activity. Saying "E-K-G" removes that ambiguity in a noisy clinical setting. Written documentation, professional bodies, and international sources more often use ECG. Neither is more "correct." They're two labels for one test, and which one dominates depends on where you are and whether you're speaking or writing.
Does it matter which term you use?
Clinically, no. Using EKG or ECG changes nothing about the procedure, the electrodes, or how the tracing is read. What matters for your work as a medical assistant is consistency with your own facility: chart it the way your office charts it, and match whatever term the provider's order and the equipment use so nobody has to wonder whether two different things were done.

What an EKG (or ECG) actually records

Where EKG fits on the CCMA exam
Recording an EKG is squarely part of a clinical medical assistant's job.
